Privacy Policy – Taylor Made Outcomes
Our commitment to your privacy
Taylor Made Outcomes is committed to protecting your privacy and handling your personal information responsibly, in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. This policy explains what personal information we collect, how we use and share it, how we keep it secure, and how you can access or correct your information.
What information we collect
We collect the personal information we need to provide your Plan Management, Support Coordination, or Psychosocial Recovery Coaching supports, including your contact details, NDIS plan and funding information, and details about the supports and services you receive. We only collect what is necessary to deliver your supports and meet our obligations under the NDIS.
How we use and share your information
We ask for your consent to collect and use your personal information when you sign a Service Agreement with us. For Support Coordination and Psychosocial Recovery Coaching, we also seek a separate NDIS Consent to Share. We only share your information with other providers, family members, carers, or NDIS bodies where this is necessary to deliver the specific support you have engaged us for, and where you have given us consent to do so.
Keeping your information secure
We store your personal information in secure systems, and access is limited to staff who need it to provide your supports. We take the following steps to protect your privacy:
- We do not record or store audio or video footage of participants.
- Access to your records is restricted on a need-to-know basis, according to staff roles.
- Electronic records are stored in secure systems with appropriate safeguards in place to protect against loss, misuse, or unauthorised access.
If there is a data breach
If a data breach occurs that is likely to result in serious harm, we will notify the Office of the Australian Information Commissioner (OAIC) in writing within 30 days, in line with the Notifiable Data Breaches Scheme. We will also notify any participants or stakeholders affected, by email or by post, according to their preference.
Accessing your information or making a complaint
You can ask to access or correct the personal information we hold about you at any time by contacting us on (02) 4208 0442 or reception@taylormadeoutcomes.com.au. If you have a concern about how we have handled your personal information, you can make a complaint to us directly, or to the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au. This policy is reviewed regularly to keep it up to date.
